This could be a very good episode if the script writers had taken some time to meld things properly. The problem is all the loose ends that have been mentioned by other reviewers. I agree that if that car trip was simply to find out where the maestro lived, what a waste of a huge chunk of the episode. Padding? Perhaps! The whole business with the elevator and the access door could have been worked out much better. The silliness of the two "witnesses" was totally ridiculous. It would have also been interesting if the conductor had trapped himself with his own words, which is what Columbo was so good at. The lack of a scream was a good observation. Anyway, the opportunities were there but were not used well.
