This episode packs a lot into 40 minutes. You have the horseman storyline, Dean being empty inside, Sam remaining addicted to demon blood, and the big revelation that John/Mary were heavenly arranged. Each of the various plot beats seems like they should be in the same episode but Edlund manages to blend each so that it all fits. This is such a key episode for the show and season overall and it's frankly amazing how much it packed into it. And it has a great gags and lines that ends up being very entertaining.
Famine was given his due and the horsemen are still the best part of this season.
