Dr. Austin guides Pravesh through placement of a transvenous pacer telling him to "access the internal jugular vein with the finder needle". The patient's chest is draped and Pravesh proceeds to place the pacer wire via the subclavian vein. The internal jugular (IJ) vein is in the neck and is generally accessed via the neck. If Dr. Pravesh uses his approach to "access the IJ" he has to thread the wire upwards toward the head rather than the heart.
