- As Shaun pushes back against Dr. Melendez in order to treat a gravely ill hospital janitor and deal with the return of Lea, Dr. Lim risks a lawsuit and her career to help a teenage girl repair the damage caused by an archaic custom. Meanwhile, Dr. Glassman exercises demanding oversight with Dr. Blaize in choosing a doctor for his brain surgery.
- On Lea's first morning in Shaun's apartment, she finds Shaun has left for work while she was sleeping.
In the hospital, Dr. Melendez is wondering why Dr. Shaun comes earlier, but Dr. Shaun just replies he just wants to get out of his apartment and work. Pointing at Paul, a janitor he met in the hospital when he tried to escape a therapist, Dr. Shaun consults Dr. Melendez on how to tell him that he has pancreatic cancer. Dr. Melendez takes a look closely and then orders Dr. Shaun for several tests without telling him the real reason. Dr. Shaun tells Dr. Melendez he was practicing lying about why he came earlier today.
Dr. Lim was surprised when a patient, Asha (Camille Hyde), requests for plastic surgery for her genital in an emergency room. Dr. Lim does an examination and shocked. Asha admitted she had circumcised a long time ago. Plans discussed with Dr. Morgan and Dr. Alex about to fix Asha's genital appearance by Labiaplasty which will be conducted the next day and finished before 6 PM as Asha do this without her parent's concern. Dr. Morgan and Dr. Alex suggest reporting the police instead because they also found Asha used a fake ID. Dr. Lim insists on her decision to do the operation anyway.
Dr. Shaun previously failed to deceive Paul that they have to do tests for the possibility of pancreatic cancer. In the MRI room, Dr. Shaun confides to Dr. Claire he is not happy with Lea's return. The screen displayed, Paul has a stage-3 pancreatic cancer and Dr. Shaun suggests a very risky Whipple Surgery. Dr. Melendez at first rejected the treatment but finally instruct Dr. Shaun to communicate his plan to Paul, teaching Shaun to do anything beyond technical in the hospital. Dr. Shaun meets Paul in the canteen and in the middle of an uncomfortable crowded place, he tells Paul has less than one year to live. Dr. Claire helps Dr. Shaun detailing the steps of the dangerous surgery in front of Paul and his family. Despite several post-surgery heavy complications, Dr. Claire offers the possibilities of a longer lifetime. The doctors let Paul has a family discussion about the offer.
The next day, as scheduled, Asha wakes up but she soon feels devastating pain in her genital. Dr. Morgan thinks it is good news exactly which shows there is still a nerve there and then there is a chance to reconstruct and make her genital normal. While reporting the circumstance to Dr. Andrews, Dr. Lim defends her decision that it was part of her effort to save the girl. The president decides to deal with administrative procedures and asks Dr. Lim to call Asha's parents soon.
Paul is in a praying room and Dr. Shaun is coming to admit he was wrong by telling Paul his dying condition. Paul said he is fine, and for him, lies do not cover up the truth but lie definitely necessary once the truth is helpless. Paul adds, his family wants the surgery and he wants what his family wants even though it will be his last. Dr. Melendez then leads the surgery and successfully remove the cancer.
An oncologist, Dr. Marina Blaize, appreciated Dr. Glassman has given nine nurses and personals for his surgery except for the surgeon. Glassman asking for a postpone but Dr. Blaize threatens within 48 hours either pick a name or find another oncologist.
Found her daughter being sedated, Asha's father (Alimi Ballard) and mother (Khalilah Joi) decided to carry Mara, Asha's actual name as unveiled by her parents, away to another hospital, avoiding further surgery proposed by Dr. Lim. Ellen Vahtra (Jordana Largy) from Child Protective Service, join the conversation and tell them Mara cannot be moved for now. In the hearing session, Dr. Lim's defended her vigilant decision on behalf of the empathy and responsibility to cure someone who severely affected by a customary tradition and desperately looking for help. Meanwhile, the couple believes that given Mara has grown as a good person, the long-practiced genital circumcise should not be a big problem.
Ellen concludes the genital cutting is an abomination anyway and Mara should pick a decision whether a riskier clitoral reconstruction which prospects to a total repair or the second option, safer and simpler treatment though permanently get rid the touch feeling off from genital organ. Mara wakes up and apparently not in Dr. Lim's favor. She feels guilty and deserved to accept the second option.
Lea visits Shaun in the hospital and asks for lunch together. Shaun refuses the offer and leaves her at once. Dr. Claire knows he refrains to meet Lea and advise him that was an unwise reply to a nice friend which Shaun said she was not.
Dr. Shaun suggests a name of surgeon but Dr. Glassman shows no interest with the conversation. Dr. Glassman then confesses, what he most worried about actually is a middle ground, a condition which patient barely life, not die but with horrible side-effects after brain surgery.
Right before the surgery, with Dr. Andrews's approval, Dr. Lim speaks privately to Mara that she has a chance for a bright future sexual life. Mara cites family and tradition but then refuses further talks without giving an objection over Dr. Lim's offer to do clitoral reconstruction. Dr. Lim assumes she already gives a verbal consent to prefer the reconstruction instead.
A complication after Paul's previous surgery leads him to a critical condition. Dr. Melendez's team is rushing to save him but Paul is helpless. Paul's family blames each other and Dr. Shaun calms them down telling it was really Paul's wish. Leaving the family grieving, Shaun quotes Paul, when the truth helpless then go lie.
Mara wakes up as the surgery went success and Lea wakes up as she hears someone prepare for breakfast. Shaun puts Lea's baseball beside her. He wants Lea to take it back and leave the apartment and go somewhere. He said he was hurt looking at the ball every day without Lea being there and now she has come back and of course, when she moves, the hurt cycle starts over. Shaun leaves Lea and goes to work. Dr. Glassman is lying on the surgery bed and obviously, he picks Dr. Ko.
