He was in the hospital for something else and knew it wasn't going to get better and would cost a lot of money, so he told Claire he wanted to die. She was supposed to give him a dose of morphine, which he had been given for his pain, and give him more than she supposed to and leave, but she stayed with him and got caught by a nurse. Because there was o proof, she was granted the time off that led her to go to England and back to Jamie.
