- Daya Zhang is touring factory locations in Mansfield, Ohio for her father's company, Hong Kong-based Zhang Innovations, Daya tells her father that bringing jobs into America will help their world wide brand and would also help uplift Mansfield, which is a dying city. Due to her passionate plea, Wu Zhang agrees. Daya Zhang (Malese Jow) and her entourage are abducted by terrorist (at the private airfield from where Daya was supposed to take off) despite her bodyguard and head of security Bao Yung (Harry Shum Jr.) attempting to fight them off. The kidnappers leave Yung unconscious with a USB flash drive addressed to security expert Ray Breslin (Sylvester Stallone).
In Los Angeles, Breslin crosses paths with Shen Lo (Max Zhang), a former Zhang Innovations bodyguard. Both men are after Daya's father Wu Zhang (Russell Wong), whose company is responsible for building covert prisons around the world. Wu Zhang has hidden the evil part of his empire from his daughter. Shen Lo defeats almost all of Wu Zhang's security before being intercepted by Breslin. Shen and Ray meet with Breslin's associates - his girlfriend Abigail (Jaime King), Hush (Curtis Jackson), and Jules (Lydia Hull). Shen says that Zhang has built high technology prisons all over the world and now makes money by torturing his prisoners on behalf of their enemies. Shen reached out to Breslin because he knows that Breslin's partner Lester Sr, took funding from Zhang. Lester, Sr. betrayed Breslin by selling out Zhang, and was sent to his death. Shen says that he has a personal vendetta against Wu Zhang and is determined to bring him down. This is when Yung arrives with the flash drive. It contains a video message from Lester Clark, Jr. (Devon Sawa), Daya's kidnapper and the son of Breslin's former partner. Breslin decides to go after Lester Jr, and Shen decides to come along as he was very close to Daya.
Breslin reaches out to Trent DeRosa (Dave Bautista), an associate, and they trace the video to a prison compound in Latvia known as "Devil's Station". Before Breslin can leave with his team for Latvia, Abigail is also abducted. Wu, having arrived in Mansfield to meet with police, receives a video call from Lester. Seeking revenge for his father's downfall (Lester claims that Zhang could have helped save Lester Sr, but he didn't. Instead, he took over all of Lester's black sites by installing his technology), Lester demands a $700 million ransom, and executes a hostage. Daya is imprisoned in the bowels of the Latvian prison site. Abigail is also brought to Lester, who claims that his father built Breslin's company, and he pushed him out.
Breslin, DeRosa, Jules, Shen, and Yung depart for Latvia to rescue Daya and Abigail. When Lester threatens the prisoners and kills a hostage, Wong (Boyang Yang), Zhang Innovations' technology analyst, agrees to give Lester access to Zhang's technology. Hush surveys the prison by drone, and its thermal imaging camera reveals Lester has created his own black site. Yung is getting impatient and wants to start the attack, but Breslin says that he will not go in blind and commit his team when he does not know what he is up against. The prison site has 2 cell blocks with 40 prisoners and a smattering of guards on each level. Jules finds a weak spot in the security in the bathrooms which is connects to tunnels beneath the site.
Breslin infiltrates the compound through the sewers while Shen and Yung approach the outer walls. Alerted to their presence, Lester leaves Daya atop the wall as bait. Realizing Lester is leading them to a trap, Shen tries to hold position and wait for Breslin to infiltrate and disable the guards from the inside, but Yung charges recklessly ahead. Yung and Shen are subdued by land mines. Lester reveals the flash drive was a tracker that led his men to Abigail and deduces that Shen and Daya are in love. Lester shoots Yung and taunts Breslin by video call before ripping out Abigail's neck.
Breslin proceeds through the compound, killing several of Lester's henchmen. Shen steals a guard's stun gun as he is taken to the cells, using it to start a fire; he melts his restraints and subdues the terrorist in the smoke. He frees Daya, but they are confronted by more terrorists. DeRosa arrives, killing the terrorists with incendiary rounds. In the ensuing chaos, Shen kills Lester's remaining henchman Silva (Daniel Bernhardt) in a hand-to-hand fight, while rescuing Wong. Trapped in a shootout with Lester in the upper cells, Breslin is wounded and disarmed, but manages to overpower Lester before slit open his neck and push him off the balcony avenging Abigail.
Returning to Mansfield, Daya coldly greets her father, now aware of the true nature of his business, before leaving with Shen. DeRosa comforts Breslin over Abigail's death, urging him to forgive himself, while at the same time persuading Breslin to join him in South America; claiming that Breslin owes him one and Breslin decides to retire.
Yung is revealed to have survived but heavily wounded. He is seen crawling and stumbling through the sewers and proceeds to escape by himself.
Breslin wearing security overalls, carrying a semi-automatic weapon, drops through a central shaft from the ceiling into the tunnel sewers, just moments after Yung had already been seen arriving in to that same tunnel from a side shaft, seemingly stalking Yung.
