Sex & Nudity

  • One enemy tries to impress you with his muscles and good looks. When you use the Check option on him to get his description and a display of his stats, he says "'Check' all you want ; )".
  • You can choose to flirt with several characters in an attempt to avoiding fighting them, but the dialogue that follows is never inappropriately suggestive and is intended to be comedic.
  • In order to befriend certain characters you can 'hang out' with them or go on a 'date'. These never get anywhere sexual.

Violence & Gore

  • You have the option to attack or spare each character or monster you enter a battle with, however this is not graphic. You have the ability to avoid fighting if that is how you wish to play the game. Undertale consistently furthers its message against senseless violence in video games.
  • If you choose to kill the character Papyrus, his fight will end in his head falling off. Due to him being a skeleton, this is not graphic.
  • Many enemies and characters will attempt to kill the player, but can be convinced not to with the MERCY option.

Profanity

  • Before her boss battle, the character Undyne is about to tell the character a story, but then she gets frustrated with herself and yells, "Screw this!"
  • In the game the word bepis is several times used, but used with humour
  • Sexy is used multiple times.
  • The player will be insulted by some characters but the insults are very mild, such as "idiot".
  • Papyrus, Alphys, and the pair of characters Catty and Bratty say "Oh my God!" multiple times.
  • Undyne says hell at least once, and she and Alphys use damn at least once each.
  • Alphys uses the abbreviated term "LMAO" several times in internet status updates.
  • In a Genocide Run, threatening the character Burgerpants leads to a sarcastic joke that he cannot go to Hell since he is out of vacation days.

Alcohol, Drugs & Smoking

  • Several characters are seen at a bar/restaurant, and the bartender offers you water but says that he would never touch the stuff. This is both a reference to alcohol and a realistic statement, as the bartender character is made entirely of fire.
  • The character Burgerpants is seen smoking if you choose to speak to him.
  • A lone pile of ashen dog treats sits on the side of a forest path. When inspected, it says that someone has been smoking them.

Frightening & Intense Scenes

  • The appearance of Omega Flowey in the Neutral Run can be considered frightening and disturbing to some.
  • Characters in a Genocide Run, specifically Sans, try to make you feel bad or turmoil of what you did, in hopes to do better and not do a Genocide Run. It's turns into very dark psychological game towards the end of a Genocide Run. In other words, it all becomes very reminiscent of the second half of "Spec Ops: The Line (2012)", but much, MUCH worse.
  • While on the Neutral/Pacifist Route. There are some intense moments but these routes have a rather comedic tone to them.
  • Although a majority of the characters in the game are "monsters", this is more of a racial term and most of them are non-intimidating, friendly and comedic. With the exception of clearly antagonistic characters they will only attack the player in an aggressive manner if the player acts threateningly.

Sex & Nudity

  • At the beginning of the game, you can flirt with Toriel by calling her on your cell phone, she will blush and sound nervous, and say she could pinch your cheek. If you call her mom and then flirt with her again, she'll make a surprised remark and call you an "interesting child"
  • The characters Undyne and Alphys will be in a romantic relationship with one another if certain story requirements are met. In one scene, they are about to kiss when a character interrupts them. In one of the endings, they finally do get to kiss. Both of them are LGBTQ+ since they are female.
  • Several characters can express their attraction to the character Mettaton, who in a more humanoid second form acts in a provocative manner (i.e. stretching out legs, striking poses). His fight contains a lot of subtle innuendo overall.
  • At the epilogue of a second consecutive "Genocide Run" (killing everyone and everything), your murderous intent gets to be too much for a being of supposedly absolute evil, who describes your violent nature as "perverted", noticing that you must enjoy inflicting suffering on others a great amount to play through the game in this extremely difficult mode to eradicate the population of the game twice.

Violence & Gore

  • The normal battles are mild in terms of violence, but if you choose to do a Genocide run and kill Sans, a giant bloody cut is seen on his stomach and blood dripping from his mouth. Not disturbing but intense.
  • The appearance of Photoshop Flowey is an amalgamation of human organs and bones, unnatural eyes, teeth, plants with large thorns, and a TV with a variety of distored black-and-white tormented faces. There is no blood whatsoever, but the appearance is very frightening and somewhat disturbing
  • The player has a choice of killing or sparing the characters who fight them, if the player chooses to attack them until the bosses health-bar is at 0, it will result in the death of that character who fights them and will effect the story.
  • Most other characters will quickly dissolve with a fading effect upon being killed. A fan theory suggests that the blood seen is ketchup, since the character is a skeleton known to enjoy drinking ketchup.
  • If the character Undyne is killed by the player at any point, she will start melting away when dying. No blood or gore is seen, but it isn't as quick as the dissolving deaths.
  • At the end of a Genocide playthrough, the player kills the character Flowey (who is a flower and bloodless as a result) in an aggressive and violent manner, slashing him with a knife until his face splits apart, and he is torn into loose shreds and beaten into the ground. This entry applies also to the Frightening/Intense Scenes category.

Profanity

  • In a Genocide Run right after Sans' first attack, he says the kid who did the Genocide should be burning in hell.
  • If you beat the final boss of a Neutral Run and then reload your save file to fight it again, Flowey gets irritated at you and will say "You're pissing me off".
  • A monster in the True Lab area says "welcome to my special hell".
  • At the end of a Pacifist run, Toriel says "Who the hell is Sans?". Everyone gives her a quizzical look, and she corrects herself, saying "I mean, who the heck is Sans?".

Alcohol, Drugs & Smoking

  • Answering "Yes" to Sans' prank phone call results in the line "Great, I'll bring over the brewskis." This does not occur.

Frightening & Intense Scenes

  • Photoshop Flowey's design is seen for a long period of time and can be frightening to some people.
  • If the you try to get the True Pacifist ending after getting the Genocide ending, the ending is altered with either a jumpscare as the player sleeps in bed or a picture of you with your friends altered to have their faces crossed out with red X's.
  • There is one jumpscare in a Genocide playthrough. Specifically, at the end, if you choose the "DO NOT" option.
  • In the True Lab section of the game in a Pacifist playthrough, you encounter monsters who have disturbing appearances and speak in a cryptic manner. The True Lab is one of the most unnerving parts of the game (using tropes common in indie horror games), and a good point to have small children avert their eyes.
  • The appearance and actions of the character Flowey are unsettling, especially his breaches of the fourth wall and Flowey's fully powered form, mainly known by the fans as "Omega Flowey". In this form, he has a scary mutated venus flytrap appearance with a TV monitor for an upper body which displays an unsettling cartoon face or a frightening realistic human face in contrast with the docile pixelated style of most other characters. However, the back half of the boss fight focuses more on a hopeful atmosphere, debatably making the fight as a whole more intense than disturbing.
  • While most of the time, Undertale strives to maintain a lighthearted and relaxed attitude. Other atmospheric changes include: realistic emotions in death scenes, slowed-down and creepy variations of songs from the soundtrack and save points which remind you of how many main characters are still alive.
  • At the end of a Genocide Run, Sans talks about "punishing" you for making the choices that you made to hurt people, and it is said that you can "feel your sins crawling on your back". This has a dark and disturbing tone to it, but is done so that the player can think about why they did what they did, regret their actions and be forgiven and accepted again by the characters in restarting the game as a better person.
