There is a scene that shows from a distance an orca being masturbated by two trainers to collect his sperm. The orcas genitals were shown close up.

Also dead orca's are seen from early in the film as they die during capture.
A trainer is grabbed and dragged under water and he then fights to get out and away from the whale. His injuries after this.
While no deaths are shown or videos of the attacks that causes the death of trainers, they are graphically described via autopsy and witness reports. For example, the film describes one of Tilikum's victims as having had his genitals eaten by the orca.
Photos of wounds inflicted on Tilikum by other captive whales are shown. At one point, video depicts Tilikum bleeding in the water. As well as wounded female who then dies due to wounds caused by other whales.
Orcas are shown screaming while being restrained for an endoscopy.
A person is attacked when an orca grabs her foot, wounds shown in a video taken of the event.

The documentary is very serious and may depress animal lovers due to its subject matter.
The descriptions of orca attacks are disturbing, as is footage of orcas being captured.
Three times in the movie it does refer to trainers being killed by the orca and two videos are shown with the orcas pulling trainers under the water
