Things have gotten wacky on the island and the creators have gotten wacky with the storytelling devices.
The show toggles back and forth between before a chemical attack and different points within it. We're going to tell it to you straight.
A group of special forces guys somehow got word to Serrat: if you poison the water supply with a military grad drug that makes people act nutty, hallucinate,and eventually fall asleep for 8 hours or so, will come in, do our business, and when we are done the men on the submarine will no longer have a reason to stay and you can have your little island fiefdom back.
Serrat does as he is told and people start wigging out.
On the island this seems to manifest itself to everyone going to the river for a drink and passing out, including many sailors.
At the NATO station first Nigel freaks out. He rudely shows Kendal a picture of his wife and Paul hugging on a tabloid gossip website. When Kendal gets chesty Nigel topples some servers and generally effs everything up. Chaplin had asked him to get a video feed going so he could talk to Sec. of Defense Curry about the SEALS. Before the chemical attack, the injured SEAL was finally up and about. Chaplin, Kendal, and Shepard went to talk to him but he wasn't giving up any info. Chaplin intended to lie to Curry and say the SEALS wanted to go public with their story. He figured he could smoke him out.
Then Nigel went all haywire and they realized something was up since Kendal was also feeling a little funny. Shepard ID-ed the drug as BZ and listed off the symptoms: hallucinations, sleepiness etc. It turns out epinephrine counters the effects. Chaplin decides that he, Shepard, and COB will go back to the sub and dole out the little epi pens that they have and babysit the crew until the island is safe again. He tells Kendal to get the epi from the hospital and hold down the NATO station.
Unfortunately, this isn't an option as all of the epi has already been stolen...by Serrat. Kendal doesn't know it yet but he's stealing them for the bad guys. He stumbles up into the hospital and sees that two of the SEALs are dead which leaves just James and Hopper, the formerly injured one. Serrat says he was dead when he found them and then tasers Kendal.
Up on a cliff Tani and James are taking a walk when she starts to hallucinate and almost falls over. James catches her and spies the special forces guys in a motor boat speeding towards the island.
Meanwhile, Sam, who wakes back up in the hospital but is still confused and is seeing Christine everywhere and she's nagging him about never coming home. He says several conversations with her. He runs into another SEAL but is confused since he doesn't recognize him. He's one of the special forces guys and he bluff his way through it, claiming to be "McClure from supply." He gets Kendal to take him to the NATO station, send a signal to the sub, and try to wake up Sophie. When this doesn't work he sends Kendal out and stabs Sophie with an epi pen.
When he comes back out Kendal is wary of him -- his vision of Christine just told him to not trust him also-- and he starts sussing him out. When he realizes he is not who he says he is he attacks him and starts beating the crap out of him, asking who he is. He incapacitates him and ties him to a chair. He then has another vision of Christine and ends up making out with her, except it's not her, it's Sophie. He doesn't realize it but she does. In the process of kissing her he smudges a little of the blood from his forehead onto hers. He then stabs himself with an epi pen that McClure had and goes off in search of Serrat.
He runs into James who is also looking for Serrat. They return to the NATO station and spy on the control room where Sophie is trying to help them under duress. Serrat gets angry at them yelling at Sophie. They send her off to reboot the power while he interrogates Hopper wanting to know where something is. Hopper says he doesn't know. James and Kendal corner Sophie, kill the guy sent to watch her and then follow her back into the control room and start firing. They mean to keep McClure alive but Serrat shoots him. Serrat explains what he did but says he doesn't know what McClure was seeking. They let him go unharmed. Sophie gets things back up and runnnig and Kendal starts trying to raise the Colorado.
Things have gone pear-shaped there as well with all of the crew dropping like flies. Shepard, COB, and Chaplin try to stay with it but it's a struggle. Suddenly alarms go off, there's a fire and it needs to be put out or their oxygen will be depleted. Chaplin gives Shepard the Conn and goes to put out the fire. He's distracted, however, by a vision of his dead son Jeffrey as a little boy. He goes to where the fire is and the COB is conked out. He starts to tell a story to the vision of his son and passes out himself. He wakes up with a syringe in his chest and sees that the fire has been contained and the oxygen is back on line. He heads back up to the Conn and answers Kendal's call saying everything seems to be okay.
Back at the NATO station things are coming back to life. Kendal tries to talk to Sophie but she is skittish. He notices the blood smudge on her head. She shrinks back and says moral lines have been crossed. He doesn't know what she's talking about because he doesn't remember.
Everything returns to normal but in the course of changing his shirt Chaplin realizes that whoever stuck him with the syrine stole his firing key. He tells Kendal they no longer have nuclear capability without the key and must get it back. He also thinks this means there's a mole on the Colorado.
Sophie is in her office when a recovered Nigel comes in to apologize. She says he wasn't himself. He looks at her laptop and asks what she's working on. She says the cameras stayed on throughout the ordeal. He wants to see. She says to wait until tomorrow. He leaves and she opens the laptop and plays back the moment that she and Kendal were making out.
On top of the station James asks Hopper, basically, what the hell is going on. He says they went into Pakistan to extract an individual but ended up killing him. Hopper says they got new orders to eliminate instead of extract. James says he didn't get those orders. Hopper says he did.
