A CIA agent is put in the ranks of Makarov, but after being killed during a terrorist attack, the Russian government believes the US ordered the attack and invades the US in retaliation. The story focuses on a US Ranger that takes part in repelling the Russian invasion, while a multinational task force seeks to find Makarov and put an end to any further damage.—Sam
- Even with the death of Imran Zakhaev in 2011, the Ultranationalist forces have conquered Russia under the command of Vladimir Makarov, protégé to the late Zakhaev, whom is now a martyr to the Russian people. In 2016 U.S Army Rangers in Afghanistan are training the locals in firearm efficiency. Sergeant Foley selects Private 1st Class Joseph Allen to demonstrate, then Allen is directed to Corporal Dunn to run an obstacle course, overseen by Lieutenant General Shepherd.
Afterwards an alarm is sounded and the Rangers are called into battle. Rangers are called to rescue a team pinned down by enemy forces, once the soldiers are saved, Shepherd recruits Allen into his new multinational force, Task Force 141. In Kazakhstan Captain John "Soap" MacTavish and Sergeant Gary "Roach" Sanderson of Task Force 141 are sent to infiltrate a Russian airbase and retrieve a satellite module containing high-value data. The module is retrieved and after a daring escape on snow mobiles, the British soldiers are extracted.
Joseph Allen has infiltrated Makarov's Inner Circle under the alias "Alexei Borodin" and is joining him for a false flag operation, at Zakhaev International Airport in Moscow, the Inner Circle massacre civilians disguised as plain-clothes C.I.A. operatives. With such a catastrophe in their wake, they leave in a van, but Makarov shoots Allen knowing he was an American spy and his presence will ensure Russia will blame the U.S.
In Rio de Janeiro, word of the massacre and rising tensions in Russia have reached 141, Sanderson, Soap and Simon "Ghost" Riley are now in Brazil to arrest an arms dealer, Alejandro Rojas, who supplied Makarov the weapons for the attack. Rojas is captured and extraction is requested, the request is denied as the United States is now under Russian invasion. The Russians replaced the satellite module with a duplicate confusing NORAD with an advance warning of an attack on the U.S west coast, the invasion is actually happens on the east. U.S Army Ranger Private First Class James Ramirez of the 1st Battalion/75th Ranger Regiment is sent to Virginia with Sergeant Foley to repel the Russian attack.
A heavy battle occurs, the Russian assault is defeated and Rangers are now called to the community of Arcadia. In Brazil, 141 have interrogated Rojas themselves for information on Makarov's greatest foe being held in a gulag. The 141 are temporarily trapped due to the war in America and the Brazilian militia surrounding them, Soap contacts Nikolai to collect them.
In Virginia, the 75th regiment are pushing through Arcadia to destroy enemy AA batteries to allow civilians to escape, after securing the evacuation zones, their next mission is to locate a VIP, said person is already dead, a briefcase full of papers is near the body, as is the body of a Russian, Viktor, one of the perpetrators of the Moscow massacre. 141 and U.S Navy Seals are neutralizing Russian occupied oil rigs, they're to disarm the Surface-to-air missile launchers protecting the gulag and rescue hostages. After completing their mission, 141 head for the gulag while U.S Marines take control of the rigs.
141 infiltrate the gulag and rescue the priority prisoner, who turns out to be Captain John Price, 141 flees the gulag with Price as the U.S Navy bombards the building. The Russians have advanced as far as Washington D.C, while the 75th are pushing them back with all they have. Foley and Ramirez board a Black-Hawk helicopter which is shot down, Ramirez holds off an assault with the on-board minigun when a bright flash blinds him. Meanwhile, in Petropavlovsk, Russia, the 141 have infiltrated a submarine dock. Price boards a submarine and launches a ballistic missile at Washington. This is intentionally to detonate in orbit, the resultant EMP blast cuts all power to the D.C area including Russian vehicles.
The 75th are still alive, they regroup at the Eisenhower building. The 75th press on to the White House, the only building with power, which is under Russian occupation, however, the Americans can reopen contact with the Central Command there, the 75th retake the building and light green flares to ward off an American air raid. The 141 have determined Makarov to be hiding in one of two places, Sanderson and Ghost are sent to a safehouse on the Russian-Georgian border, Soap and Price head to an Afghanistan boneyard.
Sanderson and Riley break into the house and find a veritable trove of information, Sanderson downloads all the data to a drive as the Task Force drive off attacking enemies. With all the data on drive, they leave, Shepherd has arrived, he takes the drive and shoots Sanderson and Riley, then his private troops, Shadow Company, burn their corpses. In Afghanistan, Soap and Price have learned of Shepherd's treachery, they are unable to contact their K.I.A allies, yet venture on even though 141 has been disavowed.
Nikolai flies them to Shepherd's base in Afghanistan, Price and Soap chase Shepherd in Zodiac speedboats to Shepherd's Warhorse 5-1 helicopter, which Price shoots down, but their boat goes over a waterfall. Soap pushes on alone, Shepherd stabs him in the chest, he explains his motivation for betraying them was due to losing 30,000 Marines in the nuclear blast five years prior and the world's inaction, continuing the American-Russo war would guarantee all Americans unite against a common enemy, Price tackles him and they fight, Shepherd pummels Price until Soap removes the knife from himself and throws it into Shepherd's eye.
Nikolai arrives and he helps Price carry Soap onto a helicopter to be treated for his injuries.
