This episode is Maura Tierney's final appearance as a main cast member. Her departure means that, besides guest appearances by former cast members, all main cast members from the show's first nine seasons are gone except for the nurses played by Deezer D, Yvette Freeman and Laura Cerón. Parminder Nagra, now the most senior cast member playing a doctor, joined the show in 2003 at the beginning of the 10th season.
Six of the twenty-three names on the wall of locker name tags represent seven producers, writers, directors, and a key make-up artist (L. Woodward, N. Baer, S. Gemmil, B. Arrollo, J. Orman, and Salamunovich - the latter representing both Mike Salamunovich and Teresa Salamunovich). The rest are names of characters, mostly doctors (Del Amico, Barnett, Finch, Benton, Corday, Doyle, Lewis, Ross, Gallant, Chen, Pratt, Malucci, Greene, Weaver, and Romano) with one nurse (Hathaway) and one medical student (Knight). Nurse Haleh Adams stated that some had to be put up for them due to their deaths. (Romano, Gallant, Greene, Knight and Pratt) Lewis' tag was taken off by her and handed into the admit desk, so it is assumed her's was put up by someone else. Weaver, Ross, Hathaway, Barnett, Corday, Doyle, Del Amico, Ross, Finch, Benton or Chen are not ever physically seen removing their locker tags. Malucci is seen taking his name tag off his locker, and taking it with him. Which is a question as to how it got onto the wall. Carter is said not to have put his locker tag on the wall because he thought it defaced government property.

The theme heard throughout the episode is a reworked version of the theme from Orion in the Sky, the farewell episode of Dr. Greene.
When Dr. Banfield refers to one of the players in the ER as Meadowlark this is a reference to Meadowlark Lemon a legendary player for the Harlem Globetrotters exhibition basketball team.
