Although this episode takes place against the background of a strike by nurses at PPTH, this was the first episode that followed the resumption of production following the 2007-08 Writers Guild of America strike, which halted production of many US TV shows, including House MD.
The episode title is the name of a song by Alice Cooper. In the song, the nice guy is also considered sick. But the expression long predates the song. Etymologists have traced its usage back at least as far as a question asked of President Nixon in 1977, and suspect that it originated at least as early as the 1950's, with similar expressions dating in the 1940's.
During the scene where House is in his apartment and the doorbell rings, when the solicitor starts his pitch, House says, "Is Miley Cyrus playing The Spectrum?" The Spectrum is an old arena in Philadelphia that opened in 1967 and closed in 2009. It was the home to the Philadelphia 76ers basketball team as well as the Flyers hockey team. Countless bands also performed there during its stay in south Philadelphia.
When House says, "Then you can have it tonight. The L Word is on," he refers to the drama series on Showtime that portrays the lives of a group of lesbian and bisexual women in the trendy Los Angeles-area city of West Hollywood.
