As is explained early on in the film, Jack is given a cuckoo clock to replace the heart he has at birth, which has frozen. Unfortunately, although it allows him to live, the substitute heart isn't as strong as a real one, so too much strain on it, as the seat of all emotion, will result in heart failure and death.
