Hamburg-schooled German expat MD Dr. Matthias Hellberg runs a practice for tourists and locals at Italy's glorious Lake Garda for years. He patiently helps people like Ingo Riedel, who can't embrace love feeling pointlessly guilty over his fiancées drowning, or Gero Schröder whose bitching wife snaps their daughter isn't biologically his. Matthias's own life is complicated when young orphan Luca's guardian-grandpa Giorgio Gabetta has a cardiac crisis and the doc's Hamburg-college-ex Bettina Stahl becomes a local hotel's new manager.

Dr. Ben Berger cheerfully runs a medical practice in Dürnstein, a town near the grand abbey of Melk in Austria's Danubian wine region Wachau. He volunteers to personally take in young Tobi Kern, who has measles, so his ma Verena can't keep him aboard the cruise ship she works on. Monk-teacher Arndt calls for Ben's help after the near-suicide of promising boarding student Lukas, whose heart was just broken, probably the wrong reason to plan joining the order. Tobi's dead Manfred picks him up but neglects Ben's prescription. He and Verena wrongly assume Ben to be married with pharmacist Karla, hence suppress their romantic interest.
