According to Dr. Cameron, they will need "a cunning plan" to help House. This was Baldrick's catchphrase in all four seasons of Blackadder. Hugh Laurie was a series regular in the series Blackadder the Third (1987) and in Blackadder Goes Forth (1989) and he also appeared in two episodes of Blackadder II (1986).
In addition to a biblical brother reference, the title can be read as a pun. During part of the show House uses a cane, and at other times he is able (to walk).
During the first differential, Foreman tells House, "You are 0 for 1 since you came back." Technically, as far as anyone in the room knew, House was actually 1 for 2, having properly diagnosed one of his patients with scurvy in the previous episode.
After Cameron (Jennifer Morrison) says, "Maybe he cheated," House (Hugh Laurie) responds later by saying, "I've no idea what you meant, but I can smell what the rock was cooking" with his eyebrow raised. Dwayne "The Rock" Johnson famously uses that line in the WWE with his signature people eyebrow.
According to the Bible story, Cain and Abel (Genesis 4:1 through 4:16) are brothers who each offer a sacrifice to God. Abel's sacrifice is favored because he gave the best part of what he had, while Cain only gave what he had left. Cain murders Abel, presumably out of jealousy. God then curses Cain to wander the earth. Cain implores God's help because he fears his fellow people's vengeance. God then puts the Mark of Cain upon him so that he shall not be killed, for "whomsoever kills Cain, vengeance shall be upon him sevenfold."
