When Donna is being shown an Israeli setters' home, it's clearly an American stucco-on-wood-frame "ranch home" with an electric service entrance and gas meter of the kind used in the Los Angeles area. An Israeli home would be made from cinder block and concrete, especially anywhere near (or in) the Gaza Strip.
When Josh visits Donna in Germany he speaks with her surgeon. The surgeon explains her injuries and tells Josh that she had suffered a pneumothorax (collapsed lung). Josh reacts like he does not know what this is. Josh when shot in the finale of season 1 suffered a pneumothorax. Josh would have been well aware of what this is and would not have needed it explained to him.
