J.D. calls Mr. Bober, the elderly man who keeps getting shuffled back and forth from his nursing home to the hospital, a "gomer." When J.D.'s brother Dan asks what a "gomer" is, J.D. explains that it's an old person that just keeps taking up a bed because he doesn't have the decency to just die already (showing that J.D. has become jaded and cynical and inspiring Dan to confront Dr. Cox about being a better role model for J.D.). Mr. Bober was the same patient in the Season 1 finale that caused J.D., Elliot and Turk to realize that they had become cynical over the past year due to the horrible way they had treated Mr. Bober (passing him off to one another, playing rock-paper-scissors in the lobby to determine who gets "stuck" with him and almost having his granddaughter overhear them and finally realizing that even though they had all had him as a patient, they didn't even know what he looked like).
Early in the episode Zach Braff (J.D.) can be seen wearing a red and black Flash t-shirt. Tom Cavanagh who plays his brother in this episode, plays Dr. Wells in The Flash (2014) TV show.
