- Graphic depictions of Hell are seen more frequently than the earlier games, including pentagrams, inverted crosses, blasphemous altars, and every stage after the eighth level takes place in Hell itself.
- A lot of the enemies can look disturbing to some players. This is due to how realistic they look compared to the game's prequels.
- Unlike the previous games, there is no soundtrack but instead extremely disturbing noises such as murmuring, chanting, and babies crying. Much darker and more horror based.
- The ending may be scary to some people.
- Compared to earlier titles, this game is quite creepy and almost all of the levels have unsettling music and atmosphere.
