The cars that crash down with the bridge are removed via crane towards the end of the film. Moments later, a character climbs down from the bridge to the railway tracks, and the cars are back where they were behind him.
The same platform and extras are clearly used for multiple pre-crash stops (notably Mt. Victoria and Medlow Bath).
The same albino-looking man in a gray suit boards the train at Parramatta twice, seen in the foreground and background as two characters board, several shots apart.
In the scene where the train led by locomotive 4620 is heading towards Granville, it can be clearly seen that both pantographs are down on the locomotive. The train would not be able to run with these down.
Boris Osman's accident site diorama features Matchbox toy cars of post-1977 vehicles, some of which weren't produced by the company until at least 1992.
In one shot of the Gordons driving to the railway station, a 1985 Ford Falcon can be seen in a background side street.
The train location is out of sync with the story line. One shot shows it to be around Emu Plains, next shot shows it to be still in the mountains. The locations are also wrong. One scene shows flour mills in the background that are located near Homebush on a freight line, well past Grandville on the line to the city.
