- During the last two years of her life, Princess Diana embarks on a final rite of passage: a secret love affair with Pakistani heart surgeon Hasnat Khan.

- The film narrates the events when Diana (Naomi Watts) divorces Charles, Prince of Wales, she falls in love with Pakistani Heart Surgeon, Dr. Hasnat Khan (Naveen Andrews). She tours Angola, Australia, Pakistan, New York, Bosnia, Italy Paris, France. She was refused by Khan and began to date Egyptian Dodi Fayed which lasts until their death in a car crash at Pont Alma Tunnel, at Paris, France on 31st Aug 1997.
Diana is formally separated from Charles in 1992. By 1995 they have been living separately for 3 yrs. As she is not yet legally divorced, Diana is called upon by the palace to attend official engagements but is allowed to see her sons Harry (Harry Holland) and William (Laurence Belcher) only once every 5 weeks. Patrick Jephson (Charles Edwards) is her private secretary. Diana meets Dr Hasnat when her private message therapist's husband has a heart attack & Diana goes to visit him in the hospital. Diana likes Hasnat as he treats her as an ordinary person and not as a princess. Their acquaintance grows into a friendship & Diana starts inviting him over to her palace. She had her trouble with the paparazzi from the very beginning who would hound Diana over speculations of a divorce with Charles.
Hasnat is hesitant to start dating Diana fearing that the media attention would distract him from his job. So Diana disguises herself with a wig to start dating Hasnat. On Nov 20, 1995, a BBC interview with Diana was aired in which she directly accuses Charles of infidelity with Pamela Parker Bowles. Diana is further isolated in her social circle because of this interview & her Secretary Patrick resigns on account of it. This drives Diana closer to Hasnat & she sleeps with him.
A year later Diana is engaged with anti-land mining missions in Angola & fights attempts by the palace to suppress the publicity she is trying to generate for this cause. Diana uses her celebrity status & walks across land mine cleared fields to bring the attention of the world & gather support to stop land-mine production. Diana gets her divorce from Charles. Diana's affair with Hasnat goes public. Hasnat breaks up with Diana after she tries to protect Hasnat by rubbishing the story, which ends up making Hasnat a walking joke at his workplace.
Diana goes berserk, but finally manages to get her act together & patch things up with Hasnat. To move things forward she goes alone to Pakistan to meet Hasnat's family. Hasnat's mother is critical of British role in the partition, but eventually seems to accept Diana. Diana travels to Italy to help support the cause of old age homes for the elderly. She meets Prof Christian Barnard, to whom she confides that she loves Hasnat & wants to marry him. She asks for Christian's help to find Hasnat a job outside of England where they can live their life in peace.
Diana travels to NY to auction her gowns to raise money for AIDS research. Hasnat is very upset when he finds that Diana used her influence to get him a job in Boston. They break up only to patch up again. Hasnat's uncle advises him that his mother does not approve of Diana as she is a divorcee & a Christian. Also, he feels that Hasnat cannot be a successful heart surgeon in the media attention his marriage to Diana would bring upon him. Hasnat makes his choice & chooses his profession over Diana. They get separated for the last time as Diana is devastated.
As Diana & her staff deals with the media aftermath of her separation, she receives an invitation from Dodi Fayed (Cas Anvar) to join him on his private yacht. Diana accepts the invitation & this time makes the relationship public from the very beginning. Despite her private life taking on bizarre turns, Diana continues to promote the cause of anti-land mine operations in Bosnia. Her relationship with Dodi attracts tremendous paparazzi attention who follow their every move. Diana is deeply upset. Dodi and Diana are in Paris in Aug 1997. Dodi & Diana are killed in a car crash on 31st Aug, while being chased by paparazzi, in a Parisian tunnel.
