- Earth has been silenced, and mankind eradicated by one final war. Now in the bowels of Hell, Pinhead - leader of the Cenobites - finds himself bored, tortured by his own immortality, and facing the fear that his own dark legion will eventually turn upon him. The only thing left to do? The last possible slice of sensation he can experience? To open the puzzle box himself.
- The screen opens to skeletons covered in dirt. The skies are dark and dingy and only ruins remain. Pinhead is sitting alone on an ebon throne, musing to himself.
"In the end only two sins killed mankind: greed and lust..." he muses, thinking upon how all at once everyone died in a nuclear holocaust without warning and without time to think upon religion.
For the first thousand years, there were plenty of souls to torment, though as each passes through the gates into their eternal torment the numbers grow fewer and fewer. The halls of Leviathan grow silent, and Pinhead's minions grow restless. He has managed thus far to silence them with theory and oratory though he knows this will not sate them for much longer.
He also knows that it will be millions of years until thinking creatures, creatures who can truly appreciate pleasure and pain, will inhabit the Earth again. Even if his minions do not turn on him, he can not bear the thought of millennia upon millennia devoid of sensation.
Knowing his fate, Pinhead makes one last desperate grasp at sensation and opens the puzzle box himself.
The room darkens and chains come down. The walls open and out comes his trusted minions, Chatterer and the Fat Cenobite. The two take out cruel looking knives and begin to viciously stab pinhead over and over. Pinhead, however, does not scream. Instead he gives a little moan and says "Such exquisite suffering..." before collapsing in his throne.
Chatterer and the fat one then proceed to tear him to bits with their knives, placing them flayed out upon a square pillar similar to the one Frank was placed on in the first movie. The movie then faces to black and the sound of wind rattling chains.
