- Um seine Zulassung als Hausarzt zu bekommen, muss Dr. Nice bei seinem Lieblingsfeind Dr. Schmidtke ein Praktikum absolvieren. Ein schwieriger Fall verspricht zum Glück Ablenkung. Andrea Wellenbrink steckt in Hochzeitsvorbereitungen mit ihrem Verlobten Marcus, als sie von rätselhaften Lähmungserscheinungen heimgesucht wird. Im Krankenhaus scheitert Dr. Schmidtke jedoch daran, die Ursache der Lähmungen zu finden, was Neiss auf den Plan ruft. Er verfolgt fieberhaft eine Fährte, die ihn zunächst an eine Lebensmittelvergiftung und dann an eine unfreiwillige Überdosierung glauben lässt, wobei Andreas Stieftochter Pia eine Rolle zu spielen scheint. Doch könnte auch eine psychosomatische Ursache dahinterstecken? Denn wie sich herausstellt, ist Andrea alles andere als überzeugt von ihrer nahenden Hochzeit mit Marcus. Sie leidet unter dieser Unsicherheit, die sie mit sich herumträgt, fast noch mehr als unter den geheimnisvollen Lähmungen an Armen und im Gesicht. Nice, der noch immer an die Heilung seiner steifen Hand glaubt, hadert unterdessen damit, dass seine Tochter Lea ins Ausland gehen will. Darüber kommt es auch zum Streit mit Leas Mutter, seiner Arzthelferin Charlie. Zudem hat er sich in Janne, die Hotelchefin, verguckt, die jedoch zögert, sich auf eine Beziehung mit ihm einzulassen. Unfähig, seine zahlreichen emotionalen Baustellen anzugehen, steigert Nice sich in sein medizinisches Problem hinein und überschreitet ethische und medizinische Grenzen, um das Rätsel um Andreas Erkrankung zu lösen. Nice überwindet sich schließlich, seiner Tochter Lea zu gestehen, dass er nur wegen ihr in diesem seltsamen Ort bleibt, der so anders ist als alles, was er bisher kannte, und sein komplettes Leben auf den Kopf gestellt hat. Doch manchmal, stellt Nice am Ende fest, lebt sich das Leben kopfüber einfach besser.
- Dr. Neiss und Charlie leisten Erste Hilfe, als vor der Praxis ein Mann zusammenbricht. Im Krankenhaus stellt sich heraus, dass der Patient Dr. Schmidtkes Schwiegervater ist. Schmidtke bewegt alles, um seinem Schwiegervater zu helfen und zu glänzen. Als nichts hilft, kontaktiert seine Frau Jana Dr. Neiss. Pikanterweise hatten die beiden im Studium eine Affäre - glaubt zumindest Schmidtke. Was die Situation nicht entspannt. Bei Dr. Schmidtke klingeln alle Alarmglocken. Es fällt ihm immer schwerer, sich auf seine Arbeit zu konzentrieren und nicht seinem ewigen Minderwertigkeitskomplex gegen seinen alten Rivalen zu erliegen, der jetzt sogar noch vor seiner Frau und seinen Schwiegereltern zu glänzen scheint. Und tatsächlich ist Nice ihm wieder einmal eine Nasenlänge voraus und kommt dem medizinischen Rätsel schließlich auf die Spur: Pössmann hat eine seltene Allergie, die sogenannte Vogelhalterlunge. Pikanterweise fliegt dabei auf, dass Pössmanns Aufenthalte in Dänemark einen ganz anderen Grund haben, als er angibt. Er hat - mit dem Wissen seiner Frau - eine Beziehung mit einer dänische Maklerin. Diese wohnt neben einem Gänsezüchter und hat die allergenen Gänsedaunen in ihrer Bettdecke, die Pössmanns Lunge fast zum Kollabieren bringen. Am Ende muss Dr. Schmidtke einsehen, dass seine Eifersucht reine Projektion war und seine Schwiegereltern in Sachen Liebe und Beziehung ihm in puncto Lockerheit einiges voraushaben. Nice wiederum kittet schließlich den wieder aufgeflammten Konflikt mit seiner Tochter und mit Charlie, die er in einem depressiven Moment vor den Kopf gestoßen hatte. Auch Hotelbesitzerin Janne ist er weiter nähergekommen - trotz gegenseitiger Beteuerungen, dass man keine Beziehung eingehen möchte. Als am Ende plötzlich einer seiner Finger unerwartet zuckt, kann Nice sein Glück nicht fassen. Sollte alles nur ein böser Traum gewesen sein? Kann er doch bald zurück in sein altes Leben?
- Bei einem Tochter-Vater-Angelausflug von Neiss und Lea passiert ein schlimmer Unfall. Mimi, die Leiterin des Ausflugs, wird von einem falsch geworfenen Angelhaken schwer im Gesicht verletzt. Neiss leistet Erste Hilfe und begleitet Mimi ins Förde-Krankenhaus, wo sie schnell operiert werden muss. Als bester Wiederherstellungschirurg der Welt bietet er seine Dienste an - was zuerst auf vehemente Ablehnung seines Widersachers Dr. Schmidtke stößt. Doch Schmidtke gibt klein bei, und gemeinsam mit der Chirurgin Dr. Birol steht Nice schließlich im OP und hilft mit seinem Spezialwissen mit, Mimis Nase zu retten. Ein kurzer Blick in alte Zeiten und die von Nice erhoffte Zukunft. Denn er ist überzeugt davon, kurz vor der Heilung seiner Hand zu stehen und bald selbst wieder als Chirurg tätig werden zu können. Was zu diesem Zeitpunkt keiner weiß: Mimi sollte ausgerechnet jetzt das erste Mal nach fünf Jahren ihre Tochter Fenja wieder treffen. Das sechsjährige Mädchen wuchs bei seinem Vater auf, weil Mimi durch eine zurückliegende Medikamentensucht nicht in der Lage war, sich um ihr Kind zu kümmern. Dass sie mitten in einem Sorgerechtsstreit steckt, wird jedoch erst deutlich, als sich ihr Ex-Mann Jesko im Krankenhaus eine Prügelei mit Hartmut liefert, Mimis Onkel, in die Nice zufällig hineingerät. Hartmut befürchtet, dass Jesko den Unfall zum Anlass nehmen könnte, dafür zu sorgen, dass Mimi ihre Tochter wieder nicht treffen kann. Mimi ist ratlos und verzweifelt. Hartmut bittet Nice, zu vermitteln und ein Treffen zu arrangieren, bei dem er als Arzt die Wiederbegegnung zwischen Mutter und Tochter moderiert. Doch Nice hat Bedenken: Wenn die wiederhergestellte Nase richtig ausheilen soll, ohne entstellende Narben, darf Mimi ihr Gesicht nicht bewegen - nicht lachen, nicht weinen, nicht sprechen. Also keine starken Emotionen haben. Weil niemand seine Bedenken ernst nimmt, entscheidet sich Nice spontan, das Treffen zu sabotieren. Mit dem Resultat, dass alle sauer auf ihn sind. Besonders Hartmut, der ihm seine Ruderfreundschaft kündigt. Zudem verfliegt Nices Hochgefühl, was die erhoffte Spontanheilung seiner Finger angeht, schlagartig, als die wieder zurückgekehrte Beweglichkeit von einem Tag zum anderen verschwindet. Nice verstrickt sich auch deswegen immer mehr in einen Kleinkrieg mit Hotelbesitzerin Janne um die Renovierung und Erweiterung seines Hotelzimmers. Gegen Jannes Verbot lässt Nice die Wand zum Nachbarzimmer durchbrechen. Und das Loch in der Wand zeigt: Dr. Schmidtke. Der wurde von seiner Frau vor die Tür gesetzt, weil sie seine gestresste Anspannung nicht mehr ertrug. Denn in der Klinik soll aus Kostengründen die Notaufnahme geschlossen werden, und als Chef ist Schmidtke unter anderem für die Kündigung seiner Mitarbeiter zuständig. Doch auch bei Mimi gibt es Komplikationen: Sie bricht mit lebensbedrohlichem Herzversagen zusammen. Das Team um Dr. Schmidtke rätselt, was das Herz der Patientin schwächeln lässt. Es ist mal wieder Dr. Nice, der sie auf die richtige Spur bringt. Der Klinikvorstand ist von Nice so beeindruckt, das er kurzerhand vom Praktikanten zum Belegarzt befördert wird, um mit lukrativen Beratungsjobs die finanzielle Notlage der Klinik zu entschärfen. Eine neue Situation, die für Schmidtke nur sehr schwer zu ertragen ist.

Their first, apparently unrelated cases are the beach mugging of fake amnesiac Miguel Soler and knife-murders of Pakistani immigrant Arsalan Qureshi, whose blood traces to his shifty brother Sameer, who left him dying in front of the mosque, and of junkie-harlot Tonia, who was dumped in her 'home'. As Xavi predicted, all major crime relates to tycoon and Catalan mob king El Tauró (Catalan for 'shark'), from drug and human trafficking plus pimping to real estate fraud. Xavi fears blackmail as he's photographed kissing unguided party animal Antoni. Hard-handed henchman Gerard links trails to Tauro's 'granny lieutenant' Izar, who kidnap and torture Sameer, like his brother a "Latero", illegal (beer) salesman, having tricked them from harmless contraband to drugs behind the boss's back, keeping his just-smuggled-in family hostage. Xavi must hastily try unravel it all and deal with a mole in time to prevent as many deaths as possible.
